You have two options for completing the inspection agreement.

You can fill out the form online and send it electronically to Pristine.

You can print out the inspection agreement, fill it out by hand, and then fax it to Pristine.

To fill out and send the inspection agreement electronically...

Click on the SEND ELECTRONICALLY button below. A new window will open with the inspection agreement. Fill in the blue fields. You may receive some warning messages that you are unable to save the data that is entered. This is not an issue. When you have completed the inspection agreement, click on the SUBMIT button and the report will be sent to Pristine electronically.

To print out the inspection agreement and fax it...

Click on the PRINT AND FAX button below. A new window will open with the inspection agreement. Click on the Adobe Acrobat Print button to print the report. (Do not use your browser's print button.) Once you have printed the agreement you can close the window. Please fill out the agreement and fax it back to Pristine at 217-402-9851.
